The BMW, reportedly travelling at 150 kph (93 mph), does not slow down and crashes straight into the back of the Skoda in the queue.
The Skoda was sent flying into the concrete wall of the toll booth in the accident and firefighters worked for 45 minutes to free the family from the wreckage.
All three of those inside the Skoda, named in reports as former tennis coach Daniel Dobrenov, 38, his wife Ana, 38, and their unnamed seven-year-old son, were induced into a coma and are being treated at a local hospital.
Local media report the woman and the child are out of danger, while the man is stable but still his life is still in danger.
Neither the BMW driver nor the female toll booth worker were injured in the incident, according to reports.
The alleged driver of the BMW was arrested and reports state he is believed to have been under the influence of drugs and medication at the time of the incident.
Police have filed a report of causing a traffic accident against him and will also be influenced by whether the man survives or not.
